Who can get full coverage of Freestyle Libre system?

Who can get full coverage of Freestyle Libre system?

1) children with type 1 diabetes mellitus, from the age of 4 to 18 (after the age of 18, an evaluation based on the success of treatment and an assessment of the indication for further use needs to be done)

2) pregnant women with type 1 diabetes mellitus and those with type 2 diabetes during pregnancy and undergoing basal-bolus insulin therapy (with 4 insulin doses or more)

3) blind people with type 1 diabetes mellitus and those with type 2 diabetes if they are on insulin therapy

4) patients with type 1 diabetes mellitus on basal-bolus insulin therapy (with 4 insluin doses or more), with at least three documented hypoglycemia in the last month (GUK<3.9 mmol/L) according to the blood glucose meter with the insured person and/or laboratory findings

5) patients after total pancreatictomy

6) cystic fibrosis patients treated with basal-bolus insulin therapy (with 4 insulin doses or more).

Who can set the indication and who conducts the mandatory evaluation?

The indication for the Freestyle Libre system or mandatory evaluation of the outcome of the FreeStyle Libre system application is carried out by the doctor sub/specialist in endocrinology and/or diabetology from the contracted hospital health institution. The person must be engaged in the treatment of diabetes mellitus, and obliged to explain the reason for setting the indication or continued use of the FreeStyle Libre system.

The evaluation of the outcome of the use of the system will be carried out at least once in the first three months and then after 6 and 12 months after the start of the use of the system and in follow-up once a year.

For each prescription of FreeStyle Libre system it is necessary to provide appropriate medical documentation on the basis of which the doctor from the contract hospital institution gives a recommendation to continue the use of aids, together with evidence of control.

The insured person cannot continue to have prescribed system if the expected outcome of the use of aids has not occurred, which is determined by:

1) after 6 months from the start of the use of the system, there has been no increase in TIR (time in the target range) by 5 % and/or a 1 % decrease in TBR (time below the target range)

2) at each inspection, it is not possible to establish that the patient has performed at least 8 scans per day or that the use of the sensor is not more than 80 % of the time, when glucose monitoring is active for a period of at least three months.

What do you obtain?

In the first year of the system use for unlimited glucose scanning the insured person is entitled to 25 sensors, because 1 sensor is contained in the starting set, which is the total of 26 sensors per year.

At the first prescribing, the starting set and the quarterly set are prescribed.

The start set contains an unlimited scanning device and one sensor.

WHAT DOES A QUARTERLY SET CONSIST OF?
The quarterly set consists of:

  • of 50 straps and 50 lancets for adults,
  • of 150 straps and 150 lancets for children

The insured person is not entitled to:

  • set for quick reading of blood sugar concentration, measurement strips and lancets (these are contained in the quarterly set).
  • transmitters for continuous glucose measurement and sensors other manufacturers placed on the list of Croatian Health Insurance Fund.
